The Greene County Health Assessment is currently being conducted by the Greene County Health Council.
We want to better understand the big-ticket health issues facing the residents of Greene County. If you are a resident of Greene County, please consider taking a few minutes to take part in the survey. Also please share it with your organization, patients, customers, family, and friends. Below you will find the community survey. The survey is in a digital format as well as paper. Feel free to send the digital survey to whoever you would like, just ensure that only Greene County residents are participating in the survey. You may also print the survey through the PDF linked below. All paper surveys can be dropped off at the Greene County Health Department. The Health Assessment process will last until October 30th.
