Community Health Assessment Survey

Greene County residents have an opportunity to help shape the future of health in our community by participating in the 2026 Greene County Community Health Assessment (CHA).

The Community Health Assessment is conducted every three years to identify our community’s greatest health needs and priorities. The results help guide local organizations, healthcare providers, public health agencies, and community leaders as they plan programs, services, and funding to improve health across Greene County.

The survey is open through October 31, 2026.

Please take a few minutes to complete the survey and share it with your family, friends, neighbors, coworkers, faith communities, and others who live in Greene County. Every response helps ensure our community’s needs are represented.
